Complete an online enrollment. You can find it at: www.4honline.com. (For those renewing their enrollment for the new program year, follow the steps there to update your details and get up-to-date copies of the forms to print and sign.)
Each member/volunteer must have one of each of the following forms submitted in paper to the club enrollment coordinator (Curtis). For liability purposes, please also provide a treatment authorization form to the leader of each project in which you are enrolled at your first project meeting.
I. Youth Treatment authorization form and/or Adult Treatment Authorization form
II. Ranch use agreement
Check or money order only, made out to the Homesteaders 4-H Club. Our 2015-2016 fees are $50 per youth and $20 per family. Please do not include project fees with your check. (Only checks or money orders will be accepted)
1 Child: $50 + $20 = $70
2 Children: $50 + $50 + $20 = $120
3 Children: $50 + $50 + $50 + $20 = $170
Bring your forms+check to Curtis at our monthly club meeting, or email Curtis to get his mailing address. Please submit all forms and payment at one time.
